The goal of the New Jersey Diabetes Prevention and Control is to help people with diabetes live a healthy and productive life. The program provides educational materials in English and Spanish to help reduce or eliminate the complications of diabetes. The brochures target the following areas:

The Importance of Immunizations

For a free brochure on the importance of flu or pneumonia immunization call the
New Jersey Diabetes Prevention and Control Program at (609) 984-6137. These brochures are available in English and Spanish. Remember these immunizations can save a life.

Controlling Blood Sugar

Control Your Diabetes For Life. Tips for Feeling Better and Staying Healthy

This fact sheet provides an action plan for diabetes control that includes tips for knowing blood sugar levels, reaching blood sugar goals, and maintaining blood sugar control.

This easy-to-read, 12-page brochure provides information for Medicare beneficiaries who have diabetes. The brochure focuses on the importance of self-monitoring blood glucose levels and explains the Medicare benefits that help pay for diabetes equipment and supplies.

An easy-to-read, bilingual (Spanish and English) brochure that explains the link between diabetes and heart disease for Hispanic and Latino Americans. This illustrated brochure encourages patients to take steps to control not only their blood glucose (sugar) levels, but also their blood pressure and cholesterol.
