Using Fillable PDF Forms

The New Jersey Casino Control Commission fillable* forms are PDF documents that can be viewed using Adobe's Acrobat Reader (version 8.1.2 or newer recommended).

Forms on this website labeled as "Fillable" can be saved and completed before printing. You MUST use Adobe Acrobat Reader 8.1.2 or newer to fill out and save the form on your computer, older versions will not let you save partial or completed PDF forms. If you do not wish to save the form, you can fill out the form using Adobe Reader, print it then mail it, OR you can print the form, fill it out legibly by hand, and mail it.

*Note that there are limitations on the functionality of these forms depending on the version of Acrobat Reader you are using.

How To Use Fillable PDF Forms

  1. Download/Save the PDF directly to your computer or to disk, for (1) PC - right-click on the link, then select save link as or save target as from the dropdown options, or (2) Mac - control-click on the link, then save download linked file as or save link as from the dropdown options. Make note of where the file is being saved so you can locate it later.
  2. Please Note:   Although you can fill in a form while it is displayed within your browser, we strongly recommend that you not do so. To prevent losing work in progress, avoid completing forms online within your browser.
  3. Use Acrobat 8.1.2 or newer to open the saved file. After opening do a "save-as" and name the file something that will be easy for you to remember.
  4. Select the hand tool from the Acrobat toolbar menu.
  5. Position the cursor inside a form field (it will change to an I-beam), and click to start typing. (NOTE: If the form field is a button, a check box, a radio button, or an item from a list, the cursor changes to an arrow.)
  6. After entering text (or selecting an item, check box, or radio button), do one of the following:
    • Press Tab to accept the form field change and go to the next form field.
    • Use the mouse to select the next form field.
  7. Once you have filled in the appropriate form fields, save and print the completed form.

Saving Fillable PDF Forms

If you would like to save completed PDF forms on your hard drive for future reference or to send them in an e-mail, you will need Adobe Acrobat Reader 8.1.2 or newer installed on your computer.
