The Community Health Improvement Plan (CHIP) reflects the top four health priorities within Mercer County.  The health priorities were derived from the strategic health planning process known as Mobilizing for Action through Planning and Partnerships (MAPP). The MAPP process was a collaborative effort of the Mercer County Partnership for Community Health, an assemblage of 37 stakeholders, community partners and government agencies from within Mercer County.
