The Community Health Improvement Plan (CHIP) reflects the top four health priorities within Mercer County.  The health priorities were derived from the strategic health planning process known as Mobilizing for Action through Planning and Partnerships (MAPP). The MAPP process was a collaborative effort of the Mercer County Partnership for Community Health, an assemblage of 37 stakeholders, community partners and government agencies from within Mercer County.

The CHIP is designed to be utilized as a resource document by agencies looking for comprehensive information on the health needs of Mercer County residents.  For additional information please contact Sharon McNellis-Kissel, Health Officer, Mercer County Division of Public Health, at (609) 989-6184 or sm11@njlincs.net.
