New "trim" treatment works application form to replace CP-1 form
The Division of Water Quality developed a new, "slimmer" application form for treatment works applications (sewer extensions, pumping stations, force mains, alternate design septic systems, etc.). Known as the TWA-1, this new form replaces the 22 page CP-1 form with a four-page form, complete with instructions. The CP-1 application form will not be used after May 31, 1995 (see "90 Day Construction Permit Rules being amended" in this issue).
Effective June 1, 1995, all Treatment Works Approval (TWA) requests should be filed with the new TWA-1 form.
In addition to the TWA-1 form, the division is also in the process of revising the TWA Engineer's Report (WQM-006 form) to reflect the regulatory changes adopted in June 1994 (N.J.A.C. 7:14A-22 and 23). This revised form will be available shortly.
If you need a copy of the new TWA-1 form, please call the Bureau of Construction and Connection Permits at (609) 984-4429.
