PART 1. SIGN-UP INSTRUCTIONS
The following sign-up instructions are for:

• Private Exam
• Dealer Exam
• Commercial Core and Category Exams, as listed on the exam application form (except for Category 6A, which is not yet available)
  

Note: Exam training manuals can be purchased only at County Cooperative Extension Offices.

A Dealer is fully certified after passing the Dealer Exam. A Private Applicator is fully certified after passing the Private exam. A Commercial Applicator is fully certified after passing the Core exam and at least one Category exam. For Commercial Applicators, please note that any exam you pass is only good for 12 months unless you become fully certified.

You can sign-up for a maximum of 3 exams on each exam date. For Commercial Applicators, the 3 exam maximum can include any combination of Core and Category exams. Up to 3 exams may be scheduled on one application form. There is a $10.00 non-refundable fee required for each exam you sign-up for.

1. Complete the enclosed application form as instructed. You must fill in 3 separate choices of exam sites and dates where indicated. Refer to the schedule on the back of this notice for available sites and dates. Every effort will be made to give the applicant the first choice. If none of the three choices is available, the next available date at the site of the first choice will be assigned. Incomplete application forms will be returned.

2. The required fee (check or money order) is to be made payable to "Treasurer-State of NJ". Applicants from local or county governmental agencies using a purchase order must submit an original completed purchase order with the exam applications. The "Payee" section should be completed as follows: Treasurer-State of NJ, DEP-Pesticide Control Program, PO Box 411, Trenton, NJ 08625-0411. Applicants from state agencies should use an Intra-Government Payment Voucher (IGPV). Call the Pesticide Control Program at (609) 984-6647 to obtain the seller account numbers to fill in. A copy of the approved IGPV must be submitted with the application forms. All payments must be for exams only.

Completed application forms and fees must be received by the Pesticide Control Program at least 14 days before the exam dates chosen. A COMPLETE PACKAGE INCLUDES:

1. Completed application form for up to 3 exams

2. Registration fee of $10.00 for each exam

3. Confirmation postcards indicating the assigned exam date, site, and time will be mailed to you. You must present the confirmation card at the exam site for admittance. If the confirmation card is incorrect or not received within 10 days after mailing your applications, please call the Pesticide Control Program at (609) 984-6614.

4. All exam fees are non-refundable. Exam dates cancelled due to severe weather may be rescheduled at no additional charge by calling (609) 984-6614.

5. Private, Dealer, Commercial Core and Categories 3A, 3B, 3C and 7A exams are closed book exams. All other Category exams are open book. Bring your training manuals for the Commercial Category exams (except 3A, 3B, 3C and 7A exams which are closed book) and a #2 lead pencil for marking the answer sheets. You may also use calculators.

6. Arrive on time with proper ID (driver's license, etc.). Latecomers will not be admitted.

The complete exam application must be received by the PCP at least 14 days before exam dates chosen.

PART 3. HAVE YOU BEEN SCHEDULED?

The DEP Online Report Portal allows you to check on your exam assignment. If your exam assignment has been made, you can verify where, on what date and at what time each exam has been scheduled. Old exam assignments still on file are also displayed. For those old assignments, the report will indicate if an exam was failed or if the applicant was a no-show.
