Form Categories: Who Fills What Form? Types of Forms Who must document?

Each teachers fills out
     at least one Teacher Identification Form (Forms A - E), and
    one Statement of Assurance (Form G, and Form H - for new Title I teachers)
Some teachers may also fill out
     an additional Teacher Identification Form (Forms A-E) - for additional content areas or specialties, and
     the Content Knowledge Matrix (Form F), as needed to fulfill the HOUSE standard.
