Forms: Types

Types of Forms Form Categories Who must document?

The first form type is a Teacher Identification Form, to list current qualifications, to determine whether the teacher satisfies the definition of a Highly Qualified Teacher based on the federal criteria for the particular certificate held and/or content area currently teaching. This form type is labeled A through E.
The second form is the Content Knowledge Matrix (Form F), to fulfill the HOUSE standard (if needed).
The third form type is the Statement of Assurance (Forms G and H), which is a signed teacher statement.

Purpose of Forms
To gather and maintain the data with regard to Highly Qualified Teachers at the building level.

Parent letter regarding credentials of specific teachers who have not yet satisfied the definition of a Highly Qualified Teacher should go to parents of students in Title I funded programs in mid-November.
