What is the HIV Home Care Program?
The HIV Home Care Program provides Ryan White Title II funds to a statewide network of licensed/certified home health care agencies to provide comprehensive services to individuals with HIV/AIDS who are living in their homes. The program is an adjunct to the New Jersey Medicaid AIDS Community Care Alternative Program (ACCAP). The majority of
HIV Home Care Program service providers are located at ACCAP sites throughout the state. Referrals into the HIV Home Care Program come from hospitals and from the community. There is no waiting period for clients who meet the eligibility criteria.

What are the criteria for enrollment into the HIV Home Care Program?
An individual must have a medical diagnosis of HIV/AIDS and physician certified medical (intravenous therapy) and/or chronic dependencies (compromised activities of daily living) which require registered nurse services in the home. Registered nurse services are defined as the direct provision of nursing services by a registered nurse and/or registered nurse supervision of licensed practical nurses, home health aides/housekeepers or personal care attendants in the home. An individual must also meet income eligibility criteria, have limited or no insurance, and be a resident of New Jersey for at least 30 days.

What services does the HIV Home Care Program provide?
Services are provided in the home through a case management system and include nursing, home health aide/homemaker and personal care attendant services, respiratory therapy, physical therapy, occupational therapy, speech therapy, social work and mental health services, nutritional counseling, durable medical equipment, diagnostic testing, intravenous therapy, nutritional supplements, and medical day care.

How long will the HIV Home Care Program provide services?
It is expected that individuals enrolled in the HIV Home Care Program will be linked, as soon as possible, whenever possible, to long-term reimbursement sources such as Supplemental Security Income, Social Security Disability, AIDS Community Care Alternative Program, Medicaid, Medicare, private insurance, or other sources. HIV Home Care Program services can begin immediately upon hospital discharge and continue until the individual qualifies for The location of HIV Home Care Program providers at ACCAP sites facilitates linkages with long-term reimbursement sources and provides for continuity of care.

Refer to the "Statewide Provider Network" list on the following page and contact the appropriate agency. The agency contact person will provide an application and a program brochure, upon request.
