Facility Search Help

The purpose of the Facility Search is to

• Locate, by one or more search items, an acute care or long-term care facility licensed and inspected by the Department of Health, Division of Health Facilities Evaluation and Licensing.
• View details about facilities that match search criteria.

Completing the search screen:

Complete one or more of the search options described below and click the Submit button.

• Facility Name: Type all or part of a facility name. The partial entry does not need to be the first letters of the name. For example, the entry "mount" will match Mount Holly Center, Rose Mountain Care Center and others.
   
  
• County OR Municipality:
  
  NOTE: If both county and municipality entries are provided, only the municipality entry will be used in the search.
  
  
  • County: Click the down arrow and then click the county. To clear this entry, either click the down arrow and click the blank line at the top of the list or click Reset which clears all search fields.
    
    Up to three counties can be selected.
    
    
  • Municipality: Type all or part of a municipality name. The partial entry does not need to be the first letters of the name. For example, the entry "orange" will match Orange, East Orange, E. Orange, West Orange and others.
  
  
  
• Type: Click the check box in front of one or more facility types. This is not an elimination option. If Ambulatory Care Facility and Comprehensive Rehabilitation Hospital are checked off, a facility can be one or both of these types.

To clear a prior search entry, click the Clear Entries - Start Over button and type in a new search.

To help your search efforts

• The more specific and complete the search criteria, the shorter the resulting matches.
• A partial entry does not need to be at the beginning of the facility or municipality name. Spelling, spacing and punctuation count. If in doubt, use a partial entry.
• Upper and lower case letters do not affect the search.
• If both the county and municipality fields are completed, the municipality field only is used in the search. The county selection is disregarded.
• Multiple-word entries are acceptable but extra spaces between words will affect the search.
