Locate a hospital, nursing home or other type of health care facility

Our Division licenses the following types of facilities:

Acute Care

Type of Facility Definition
Ambulatory Care Facility means a health care facility or a distinct part of a health care facility which provides preventative, diagnostic, and treatment services to persons who come to the facility to receive services and depart from the facility on the same day. (8:43A-1.3)
Ambulatory Surgery means a surgical facility in which ambulatory surgical cases are performed and which is licensed as an ambulatory surgery facility, separate and apart from any other facility license. (The ambulatory surgery facility may be physically connected to another licensed facility, such as a hospital, but is corporately and administratively distinct.)
Birth Center means a health care facility or a distinct part of a health care facility which provides routine prenatal and intrapartal care to low-risk maternity patients who are expected to deliver neonates of a weight greater than 2,499 grams and of 36 weeks gestational age and who require a stay of less than 24 hours after birth. "Routine intrapartal care" means labor and delivery services not requiring surgical intervention.
Chronic Hemodialysis means dialysis rendered to a patient with end stage renal disease in whom recovery of renal function is not expected.
Comprehensive Outpatient Rehabilitation means an ambulatory care facility which provides at least medical, physical therapy, and social or psychological services in a coordinated manner. The term applies to facilities which are certified or eligible for certification as comprehensive outpatient rehabilitation facilities in accordance with 42 CFR Part 485, Subpart B.
Comprehensive Rehabilitation Hospital means a facility licensed by the New Jersey Department of Health to provide comprehensive rehabilitation services to patients for the alleviation or amelioration of the disabling effects of illness. Comprehensive rehabilitation services are characterized by the coordinated delivery of multidisciplinary care intended to achieve the goal of maximizing the self-sufficiency of the patient. A rehabilitation hospital is a facility licensed to provide only comprehensive rehabilitation services or is a distinct unit providing only comprehensive rehabilitation services located in a licensed health care facility. (8:43H-1.3)
Computerized Axial Tomography (CAT) means an individual piece of diagnostic medical equipment which permits the imaging of internal structures of the body through the use of computer assisted x-ray scanning.
Drug Abuse Treatment means methadone detoxification, methadone maintenance, and/or drug-free counseling programs.
Extracorporeal Shock Wave Lithotripsy stands for extracorporeal shock wave lithotripsy (ESWL) or lithotripsy, which means the process by which kidney stones are pulverized into particles of sand.
Family Planning means comprehensive reproductive health care services including contraception, pregnancy detection, options counseling, diagnosis and/or treatment of sexually transmitted diseases, routine gynecological and cancer screening services, health promotion activities, and Level I infertility services. Family planning services may also include prenatal and postpartum care, other gynecological services including colposcopy and cryotherapy, menopausal services, and/or Level II and III infertility care. Family planning services do not include termination of pregnancy.
Family Planning - Satellite means an affiliate of a separately licensed ambulatory care facility. A satellite is located at a site distinct from, and within 30 miles of, that of the separately licensed ambulatory care facility, but shares the same governing authority and provides the same principal service as the separately licensed ambulatory care facility.
General Acute Care Hospital means an institution, whether operated for profit or not, whether maintained, supervised or controlled by an agency of the Government of the State or any county or municipality or not, which maintains and operates facilities for the diagnosis, treatment or care of two or more non-related individuals suffering from illness, injury or deformity and where emergency, out-patient, surgical, obstetrical, convalescent or other medical and nursing care is rendered for periods exceeding 24 hours. (8:43G-1.2)
Home Health Agency means a facility which is licensed by the New Jersey State Department of Health to provide preventative, rehabilitative, and therapeutic services to patients in the patient's home or place of residence. All home health agencies shall provide nursing, homemaker-home health aide, and physical therapy services. (8:42-1.2)
Hospice Branch means a program which is licensed by the New Jersey Department of Health to provide palliative services to terminally ill patients in the patients home or place of residence, including medical, nursing, social work, volunteer and counseling services. (8:42C-1.2)
Hospice Care Program means a program which is licensed by the New Jersey Department of Health to provide palliative services to terminally ill patients in the patients home or place of residence, including medical, nursing, social work, volunteer and counseling services. (8:42C-1.2)
Hospital, General Acute Care means an institution, whether operated for profit or not, whether maintained, supervised or controlled by an agency of the Government of the State or any county or municipality or not, which maintains and operates facilities for the diagnosis, treatment or care of two or more non-related individuals suffering from illness, injury or deformity and where emergency, out-patient, surgical, obstetrical, convalescent or other medical and nursing care is rendered for periods exceeding 24 hours. (8:43G-1.2)
Hospital-Based, Off-Site Ambulatory Care Facility means an ambulatory care service facility which has met the criteria as set forth in N.J.A.C. 8:43G-2.11 ( c) to be classified as same and which has applied for and received a license authorizing the facility to operate as a hospital-based off-site ambulatory care service facility. (8:43G-1.2)
Magnetic Reasonance Imaging (MRI) means an individual piece of diagnostic medical equipment which produces images that are based on the responses of atomic nuclei in a magnetic field. By depicting the concentration, chemical form and spatial distribution of certain nuclei, MRI can serve to define body constituents and chemistry, directly and non-invasively
Maternal and Child Health Consortium means a voluntarily formed non-profit organization, incorporated under Section 501( c) (3) of the United States Internal Revenue Code, consisting of all inpatient, ambulatory perinatal and pediatric care providers and related community organizations in a maternal and child health service region, licensed as a central service facility by the Department of Health. (8:35A-1.2
Megavoltage Radiation Oncology Services means an individual piece of radiotherapy equipment capable of generating particle beam energies in excess of 1,000 kilovolts
Positron Emission Tomography (PET) means an individual piece of diagnostic medical equipment that produces an image that provides functional and metabolic information by introducing radionucleide or isotopes with extremely short half-life's into the system and tracing its behavior.
Primary Care means the provision by a health care facility of preventive, diagnostic, treatment, management, and reassessment services to individuals with acute or chronic illness. The term is used in reference to facilities providing family practice, general internal medicine, general pediatrics, obstetrics, gynecology, and/or clinical preventive services, including community health centers providing comprehensive primary care. Comprehensive primary care may include the provision of sick and well care to all age groups, from perinatal and pediatric care to geriatric care. Primary care is further characterized by the fact that it represents the initial point of contact between an individual and the health care system, by the assumption of responsibility for the person regardless of the presence or absence of disease, by the ongoing responsibility for coordination of medical care for the person, by its family-centeredness, and by its community orientation.
Primary Care - Satellite means an affiliate of a separately licensed ambulatory care facility. Psychiatric Hospital which shall include any hospital which assures provision of comprehensive specialized diagnosis, care, treatment and rehabilitation where applicable on an in-patient basis for patients with primary psychiatric diagnoses.
Satellite Emergency Department (SED) means a facility, which is owned and operated by a licensed New Jersey general acute care hospital, which shall provide emergency care and treatment for patients.

Long-Term Care

Type of Facility Definition
Adult Day Care Health Services means a facility or a distinct part of a facility which is licensed by the New Jersey Department of Health to provide preventive, diagnostic, therapeutic, and rehabilitative services under medical and nursing supervision to meet the needs of functionally impaired adult participants who are not related to the members of the governing authority by marriage, blood, or adoption. Adult day health services facilities provide services to participants for a period of time, which does not exceed 12 hours during any calendar day.
Alternate Family Care means a contractual arrangement whereby no more than three persons receive room, board, personal care and other health care services in the home of an unrelated individual who has been approved by a sponsor agency and trained to provide the necessary caregiving.
Assisted Living Program means the provision of or arrangement for meals and assisted living services, when needed, to the tenants (also known as residents) of publicly subsidized housing which because of any Federal, State, or local housing laws, regulations or requirements cannot become licensed as an assisted living residence. An assisted living program may also provide staff resources and other services to a licensed assisted living residence and a licensed
comprehensive personal care home.
Assisted Living Residence means a facility that is licensed by the Department to provide apartment-style housing and congregate dining and to assure that assisted living services are available when needed, to four or more adult persons unrelated to the proprietor. Apartment units offer, at a minimum, one unfurnished room, a private bathroom, a kitchenette, and a lockable door on the unit entrance.
Behavioral Management Program means a program that provides specialized long-term care for residents with severe behavior management problems, such as combative, aggressive, and disruptive behaviors.
Comprehensive Personal Care Home means a facility that is licensed by the Department to provide room and board and to assure that assisted living services are available when needed, to four or more adults unrelated to the proprietor. Residential units in comprehensive personal care homes house no more than two residents and have a lockable door on the unit entrance.
Hemodialysis means dialysis therapy provided to either:
1. A hospitalized individual who abruptly sustains loss of kidney function, in whom dialysis is required as a temporary life-supporting measure and in whom recovery of kidney function is expected; or
2. A hospitalized individual with end stage renal disease who requires a regular course of maintenance dialysis therapy.
Long Term Care/Hospital Subacute means a category of special hospital that provides acute care through a broad spectrum of clinical care services for acutely ill/medically complex patients requiring, on average, a 25 day or greater length of stay. A long term acute care hospital may either be freestanding or a hospital within a hospital in accordance with the definitions identified at N.J.A.C. 8:43G-1.3(b)2.i and ii.
Long Term Care Pediatric means a facility, distinct nursing unit, or program which is dedicated for occupancy by residents under age 20.
Nursing Home means a facility that is licensed by the Department to provide health care under medical supervision and continuous nursing care for 24 or more consecutive hours to two or more patients who do not require the degree of care and treatment which a hospital provides and who, because of their physical or mental condition, require continuous nursing care and services above the level of room and board.
Pediatric Day Health Care Services means a facility which provides additional services in order to provide for the needs of technologically dependent or medically unstable children and conforms to the rules of this chapter and N.J.A.C. 10:122, the Manual of Requirements for Child Care Centers.
Residential Health Care Facility means a facility. attached to another licensed long-term care facility, that provides food, shelter, supervised health care and related services, in a homelike setting, to four or more persons 18 years of age or older who are unrelated to the owner or administrator.
Special Hospital means a hospital which maintains and operates organized facilities and services as approved by the Department for the diagnosis, treatment or care of persons suffering from acute illness, injury or deformity in which comprehensive specialized diagnosis, care, treatment and rehabilitation are administered by or performed under the direction of persons licensed to practice medicine or osteopathy in the State of New Jersey, where applicable, on an inpatient basis for one or more specific categories of patients as approved by the Department.
Ventilator Beds means specialized long-term care beds for residents who require ventilator treatment.  These beds shall have piped-in oxygen, suction equipment, emergency electrical outlets, and additional square footage availablefor ventilator equipment and supplies.
