Register a Patient

Only physicians registered with the Medicinal Marijuana Program (MMP) can authorize patients for the MMP.  Per regulation, only the patient’s bona fide physician can initiate the patient’s application via on-line registration.

A physician must first be registered with the MMP.  The registered physician will complete and submit their patients Attending Physician Statement to the MMP.  The MMP will return a Patient Reference Number and Patient Registry ID to the physician so that patient registration can be initiated.  The physician can then register their patient electronically at https://njmmp.nj.gov .

If the physician chooses to aid the patient in completing the registration process; the physician, patient and caregiver (if applicable) will need the following:

Physician

  • Laptop or PC with internet access
  • Scanner
  • Digital camera or web camera
  • Patient Registry ID and Reference Number

Patient

  • Government issued photo identification (examples)
  • Proof on New Jersey residency (examples)
  • Proof of government assistance (if applicable) (examples)
  • E-mail address

Caregiver (if applicable)

  • Government issued photo identification (examples)
  • Proof on New Jersey residency (examples)
  • Proof of government assistance (if applicable) (examples)
  • E-mail address

Patient Registration:

From https://njmmp.nj.gov click the Patient Registration tab on the left side of the page and the Patient Registration page will appear.  Enter all required patient information (as noted by the red asterisk).  The patient will read and acknowledge the Patient Certification by checking the appropriate box.

Caregiver registration (if applicable) - Enter all required caregiver information (as noted by the red asterisk).  The caregiver will read and acknowledge the Caregiver Certification by checking the appropriate box.

Once the Patient Registration page is completed with all the required information, including Primary Caregiver (if applicable); click on "Save and Continue" at the bottom of the page and a secondary Patient Registration page will appear. This page informs you that the application has been successfully submitted for review. The patient and caregiver (if applicable) will be prompted to continue to the document upload page.

Patient/Caregiver Required Documentation:

The secondary Patient Registration page requires the attachment of a photograph and additional documents necessary for the patient/caregiver registration.

Photograph of Patient and/or Caregiver:

  • The physician will capture a digital photo using a PC web camera or digital camera;
  • the photo will be taken against a white background or backdrop; 
  • the patient/caregiver will not wear a hat, glasses or any other item that may alter or disguise the overall features of the face in the photograph; 
  • the face must take up 70 percent of the picture;
  • the digital photograph must be in GIF or JPEG format; and
  • The photograph will be saved on the physicians PC or laptop.

Documents for Patient and/or Caregiver Registry:

  • The following documents (one each) will be scanned and saved on the physicians PC or laptop;
  • Proof of Identification – Government issued photo ID (examples on page 3).
  • Proof of New Jersey residency (examples on page 3).
  • Proof of government assistance (if applicable) (examples on page 3).

To upload photograph complete the following instructions:

  • From the Patient Registration page left click on the Browse tab for Patient Photo or Caregiver Photo (if applicable). 
  • The Choose File to Upload page will appear and give access to the files stored on your computer. 
  • Locate the appropriate picture from the computer files and double left click on it.  The picture file will be populated in the proper location on the Patient Registration page.
  • If completed left click on “Save” at the bottom of the page.

To upload documents complete the following steps:

  • From the Patient Registration page left click on the “Browse” tab for Document 1.
  • The Choose File to Upload page will appear and give access to the files stored on your computer.
  • Locate the file you want to upload from your computer, double left click on the file and it will be populated in the proper location on the Patient Registration page.
  • Follow the same procedure for all documents that you need to submit, using the Document 2 and the Document 3 lines as applicable.

When all documents are loaded, click on the “Save” button and all information will be entered into the Patient Registry system.

Down load the Criminal Background Check form and provide to caregiver.  Caregiver will be responsible to initiate and complete the fingerprinting process.

The MMP will review the application and supporting documents.  All applicants will be responded to via email with further instructions for finalizing your application.  Approved applicants will be instructed on the MMP e-payment process.  Denied applicants will be provided instructions on amending your application.

The patient and caregiver must each supply one (1) photograph, one (1) proof of identification and one (1) proof of NJ residency before an application will be accepted and processed.

The following examples are the ONLY acceptable documents for registration.

Proof of Identification – Government issued photo ID
The following is a list of authorized documents:

  • Current NJ digital license
  • Current NJ digital non-driver ID card
  • US passport, current or expired less than 3 years
  • Current US Passport card
  • Valid active duty US military photo ID card
  • Current photo employment authorization card
  • US military photo retiree card

Proof of New Jersey Residency
The following is a list of authorized documents:

  • Utility or credit card bill issued in the past 90 days that shows your name at your current address
  • Checking or savings account statement from a bank or credit union, issued within the past 60 days
  • Original lease or rental agreement showing your name as the lease or renter
  • Property tax bill, statement or receipt from the past year
  • Any letter or correspondence (including tax bills) received from the IRS or state tax office in the last year
  • First-class mail received from any federal, state or local government agency within the past six months
  • If you are under 18, we will accept the Parent or Legal Guardian Certifications required upon minor patient registration

Proof of Government Assistance

  • New Jersey Medicaid program
  • Food stamp benefits
  • New Jersey Temporary Disability Insurance benefits
  • Supplemental Security Income (SSI) benefits
  • Social Security Disability (SSD) benefits
