How to Transfer a Patient from another Physician

In the event an approved MMP patient requests or needs to change their doctor, the following instructions are provided.  The new approved MMP physician must:

• Log onto njmmp.nj.gov
• Click Physician Statement
• Click Patient Transfer
• Enter the Patient ID (as it is printed on card) and last name of patient in the appropriate boxes and click Search
• Check Certification box and Accept Patient
• Complete the red asterisks on physician statement
• Enter any comments
• Click Update
