TRENTON – Effective Monday, February 14, 2005, the state Office of Vital Statistics will be relocating its in-person record request services to the Motor Vehicle Commission (MVC) building located at 120 South Stockton Street, Trenton, the New Jersey Department of Health and Senior Services announced today.

All in-person requests for copies of vital records - birth, death, marriage and domestic partnership certificates - must now be obtained at this new location.  Document fees will remain the same, as will office hours, which are from 8:30 a.m. to 4:30 p.m., Monday through Friday, excluding state holidays.

The move will streamline services for the public who are obtaining copies of both MVC records and vital records.  It will also provide greater accessibility for vital statistic services, which previously operated out of the first floor of the Health and Agriculture Building located at Market and South Warren Street.  Vital Statistics’ administrative offices, however, will remain in this building.

Persons interested in requesting documents in person should note that there is up to a two hour processing time and applications must be submitted by 4:00 p.m. to obtain a certified copy the same day. Applicants must establish their identity by providing a valid photo driver's license or a non-driver’s license photo I.D. with their address. 

If any individual does not have either of these forms of identification, they may provide two alternate forms of identification, one of which must show their address.  Acceptable types of alternate identification are: driver's license without photo, vehicle registration, insurance card, voter registration card, passport, green card, county I.D., school I.D. or a copy of a utility bill.

If an applicant is requesting a record other than their own, they must provide proof of their direct relationship to the individual listed on the vital record, such as a birth or marriage certificate.
