04.17.15 New Jersey Submits HCBS Statewide Transition Plan
On Friday, April 17, 2015, the New Jersey Department of Human Services submitted the state's Home and Community Based Services (HCBS) Statewide Transition Plan to the federal Centers for Medicare and Medicaid Services.

04.13.15 Combined Application Now Open for All Services

04.10.15 Division to Assess / Re-Assess all Individuals Served
To ensure that the Division has the most up-to-date and accurate information for individuals served, everyone who is currently eligible for Division-funded services (except for those individuals assessed since November 1, 2014) will be assessed/re-assessed through the NJ Comprehensive Assessment Tool (NJ CAT). The assessment/re-assessment process will begin in April, 2015 and occur in phases over the next several months.
To assist individuals, families and providers in better understanding the NJ CAT assessment, the Division has released a pre-recorded webinar, Completing the NJ CAT.

11.21.14 2015 Graduates Timeline Available
The 2015 Graduates Aging Out of the School System Timeline
is now available. It provides an overview for young adults who will exhaust their educational entitlement by turning 21 years of age within the 2014-15 school year, and who wish to pursue Division-funded services.
10.10.14 Provider Qualification Process Temporarily on Hold
Please be advised that effective October 20, 2014
, the qualification of new service providers will be temporarily on hold
. Any applications currently in process or those in transit and received by October 17, 2014 for open services at this time will continue to be processed. The Day Habilitation qualification process continues to be on hold.
This change is necessary due to the development of the Medicaid provider application process as part of the Division’s system reform efforts. The Medicaid provider application will remain open for Support Coordination services only. Please check the Supports Program
page of the Division’s website regularly for updates regarding the Medicaid application process.

A Central Registry of Offenders against Individuals with Developmental Disabilities
was established in the Department of Human Services through bi-partisan legislative support and a law signed by Governor Chris Christie on April 30, 2010. Information can be found at Central Registry of Offenders against Individuals with Developmental Disabilities
