Records request forms must indicate the specific agency that maintains the record being requested. Denial of a request or delays could occur if a request is made to the incorrect agency. OAG-L&PS Records Custodians will make an effort to inform you of possible errors and assist you in resubmitting your request to the appropriate agency rather than simply denying a request made to the wrong agency.
