The New Jersey Department of Transportation (NJDOT) issues licenses and registrations, determines eligibility of projects for funding, determines security/search and volunteer rescue work, records accidents and incidents and looks at certain consumer issues. All of these categories require the completion of a form in order to initiate a process.

You can open the .RTF files in any word processor and input your information into the form on your computer.

Completed forms and supporting information should be mailed NJDOT Division of Aeronautics. Time-sensitive material may be faxed to 609.530.5270.
