DEPENDENT ELIGIBILITY VERIFICATION AUDIT (DEVA)
Dependent Terminations and Appeals
for DEVA Noncompliance

Following an extended submission deadline which ended December 11, 2009, the SHBP completed Phase 1* of the Dependent Eligibility Verification Audit (DEVA).

  • Beginning January 7, 2010, the SHBP mailed letters to affected members indicating that dependents will be terminated from coverage for noncompliance with the audit.

  • Termination of coverage will be on January 30, 2010 for dependents of State Employees paid through the State Centralized Payroll Unit and February 1, 2010 for dependents of employees of State Colleges/Universities, autonomous State agencies/authorities, and dependents of State Retirees.

  • If your dependents are terminated and you feel that they meet the eligibility requirements of the SHBP and you have submitted verifying documentation, you may appeal in writing to the SHBP. A detailed explanation of the reason for your appeal and supporting documentation should be sent to:

Appeals must include a cover letter with the full names and Social Security numbers of the member and all dependents along with the member's mailing address and daytime telephone number. Be sure to also include a detailed explanation of the reason for the appeal and copies of all documents required for proof of the dependent’s eligibility for coverage — For a list of required documentation, click here!   • If you have questions about the audit, the coverage status of your dependent(s), or the appeals process, please call 1-866-641-5651. When prompted for your Company Identification Number Phase 1 members (State employees/retirees) should enter 8642. Representatives are available Monday through Friday, 8:00 a.m. to 8:00 p.m. EST (excluding State holidays) and Saturday, 8:00 a.m. to 4:00 p.m EST.

The Call Center will close on August 9, 2010, at 6:00 p.m.
