Which Form to Use
Form PTR-1 is for applicants who are filing for a reimbursement the first time (or who filed an application the previous year that was denied for failure to meet all the eligibility requirements). To print Form PTR-1 for the current year or a prior year, use the links above.

Form PTR-2 is a personalized application that is mailed only to those who filed an application and met all the eligibility requirements for a reimbursement the previous year. It is not the application for a Homestead Benefit.

Form PTR-2 is not available on this Web site. If you qualify to use Form PTR-2, and you have not received a personalized application:

• Call the Property Tax Reimbursement Hotline at 1-800-882-6597 to have one mailed to you.
• E-mail your request to the Division. Please include the following information in your message: name, address, daytime phone number, name of program or form, and tax year (e.g., 2010 Property Tax Reimbursement Application, Form PTR-2). E-mail us now.
