"This form shall be used for all new or existing ISRA cases seeking an approval of an application for an alternate
compliance option available pursuant to subchapter 5 of the ISRA rules. Note: Separate forms exist for a Deminimus
Quantity Exemption, Remedial Action Workplan Deferral and Limited Conveyance."
