The following services may be available to you as an individual enrolled in Managed Long Term Services and Supports (MLTSS). All services will be approved by your MLTSS Care Manager based on your specific care needs and will be included in your Plan of Care. If you choose to receive any of these services, you will be responsible for finding the provider, organization, agency or individual to provide the service.


Chore Services are supports designed to help an individual maintain a clean and safe home environment. Chores covered by this service include:

  • Cleaning appliances
  • Cleaning carpets and scrubbing floors
  • Washing walls and windows
  • Cleaning attics and basements to remove fire and health hazards
  • Clearing walkways of ice, snow and leaves
  • Replacing fuses, light bulbs, electric plugs, frayed cords
  • Replacing door locks, window catches, faucet washers
  • Installing safety equipment like smoke detectors, fire extinguishers and grab bars
  • "Spring Cleaning" and weatherization


Service Limitations and Exclusions:

  • Routine everyday housekeeping tasks such as dusting, vacuuming and laundry
  • The participant or other household members must not be capable of performing these tasks on their own or there is no one else in the household capable of financially paying for the chore service
  • Circumstances in which another source is responsible for the completing or paying for the service (landlord, homeowners association, etc..)
  • Utility providers who offer free services for home weatherization/energy efficiency products are required to be used first
  • Individuals residing in rental properties or communities with homeowners associations will be required to get permission from the landlord or association before a service, repair or renovation will be approved. Individuals can choose to hire a licensed contractor or registered business in New Jersey to provide these services.

Home-Based Supportive Care (HBSC) services are designed to assist MLTSS participants with Instrumental Activities of Daily Living (IADL). IADLs are support services such as, but not limited to:

  • grocery shopping
  • money management
  • light housekeeping
  • laundry


Individuals can choose to hire a licensed agency to provide these services, or they can choose to self-direct HBSC services by hiring an individual of their choice to provide the service through the Personal Employer Provider (PEP) program.


Under PEP, individuals choose their own caregiver, who is set-up as a private contracted worker to provide assistance with IADLs. Workers are reimbursed a set hourly rate of pay which is determined by the program. A Fiscal Management Service (FMS) for the program will provide timesheets to participants and their workers and the issue pay checks to the private contracted worker on behalf of the participant.


Service Limitations and Exclusions:

HBSC is not available for individuals who receive Medicaid Personal Care

Assistance (PCA) services or have living arrangements where this type of service is already provided.


Non-Medical Transportation is a service which helps individuals to gain access to community services, activities and resources which enhance the individual’s life. This service is offered in addition to medical transportation. Transportation covered by this service include:

  • Shopping
  • Beauty salon
  • Financial institution
  • Religious Services


Service Limitations/Exclusions: 

  • Services are based on individual need
  • Services cannot be used for medical transportation
  • Individuals must access any other available transportation service first, such as family, friends, public transportation, community buses and local organizations.
