Question 1: The solicitation states that “Effective October 26, 2005, pursuant to Public Law 2005, Chapter 51 (formerly Executive Order No. 134), all New Jersey and out of State consultants must provide a completed, signed PUBLIC LAW, CHAPTER 51 CERTIFICATION. The certification must be submitted by the firm with its technical proposal at the time of proposal closing.”
There is a link to the NJ Treasury website, but it is not clear which form should be selected. Should we submit the Chapter 271 Vendor Certification and Political Disclosure Form, or the Two-Year Ch 51/Executive Order 117 Vendor Certification and Disclosure of Political Disclosures Form? Please clarify.
Answer 1: Include the "Two-Year Ch 51/Executive Order 117 Vendor Certification and Disclosure of Political Disclosures Form."
Question 2: Please confirm which other forms should be submitted with the Technical Proposal. The NJ Treasury website and the solicitation offer conflicting requirements. It is my understanding that the forms listed below are required, please confirm.
- Ownership Disclosure Form
- Disclosure of Investment Activities in Iran Form
- The form referenced in item Question #1
Answer 2: You are correct. The other two forms to be submitted are the "Ownership Disclosure" form and the "Disclosure of Investment Activities in Iran" form.
Question 3: In the Technical Proposal Evaluation Criteria Form (Part 4, Section 10: Experience) are respondents supposed to mark years of experience with an “x” or provide the actual years of experience in addition to that indicated in the field. The instructions indicate that the “Years of experience indicated below must be in addition to years of experience claimed in Section 1B”; however, there are no years of experience provided in Section 1 B. What is the preferred response - Example 1 or Example 2?

Answer 3: Part 4 Section 10 should be marked with an X. The resume should highlight the actual relevant experience associated with the qualifications noted.
Question 4: Instructions show Section 7 – PM past performance as being required but in Table A it is not required
Answer 4: Instructions should say “NOT REQUIRED FOR THIS TECHNICAL PROPOSAL” in Section .
Question 5: Instructions show Section 12 – Key issues and critical problems as not required but in Table A it is required.
