HCBS Statewide Transition Plan
The Home and Community Based Services (HCBS) Settings Rule, issued by the federal Centers for Medicare and Medicaid Services (CMS), states that all HCBS must be delivered to individuals in settings that are integrated in and support full access to their community. This includes opportunities for individuals to seek employment and work in competitive settings within the community, engage in a community life, control personal resources, and receive services similarly to individuals who do not receive HCBS.
