2009 TPA Reporting Requirements

Below are the 2009 Reporting Requirements for all TPAs who have a license in New Jersey.

Please note that the changes from 2008 are in bold.

Some of the major changes are as follows:

  1. Annual Statement Instructions (PDF)

  2. Annual Questionaire (PDF)

  3. Annual Contract Report

  4. Updated contact information is required no later than May 1, 2009.
