The aim of the Hunterdon-Mercer County Regional Chronic Disease Coalition is to work toward improving health outcomes for people with or at risk for cancer and other chronic diseases. This includes delivery of cancer and chronic disease prevention education, support of early detection initiatives and addressing survivor quality of life. Policy, systems and environmental change through a collective impact approach with community stakeholders addresses issues on a multi-dimensional level to achieve sustainable change.
