Systems Integration Medical Home Grant

In September 2015, the New Jersey Department of Health (NJDOH) was awarded a Systems Integration Grant (SIG) funded by the Health Resources and Services Administration’s Maternal Child Health Bureau (HRSA/MCHB) through August 2017.

The primary goal of this grant is to increase by 20% the number of children and youth with special health care needs (CYSHCN) who have a patient-centered medical home by October 2017. HRSA/MCHB has prescribed three strategies and respective objectives for each strategy that have been tailored to fit the needs of New Jersey’s CYSHCN. These include:

  1. Care Coordination: By October 2017, states will increase by 20% over baseline, the % of targeted CYSHCN who have a Shared Plan of Care
  2. Care Integration: By October 2017, an agency-level written agreement will be developed between two or more state or regional-level entities to improve the timely receipt of information following the initial referral of a CYSHCN by a medical home
  3. Shared Resource: By October 2017, 50% of families and medical home providers of CYSHCN contacting a shared resource for a needed specialist, support, or service will obtain a needed specialist support or service
