Patient Safety Dashboard

The Patient Safety Reporting System (PSRS) Dashboard provides the public with a basic overview of serious preventable adverse events and their associated root causes reported to the Department of Health from licensed health care facilities in New Jersey, as required by the Patient Safety Act N.J.S.A 26:2H-12.23-12.25.  The following facilities are currently required to report serious preventable adverse events to DOH:

All Licensed:

General Acute Care Hospitals

Comprehensive Rehabilitation Hospitals

Psychiatric Hospitals

Special Hospitals and

Ambulatory Surgery Centers (ASCs)

End Stage Renal Disease Facilities

 

This dashboard contains summary statistics on reporting patterns of serious preventable adverse events, and associated root causes and contributing factors for all reportable events.
