NJ Health Commissioner Joins NJ Hospital Association to Unveil New Jersey’s Electronic POLST Form

New Jersey Health Commissioner Cathleen D. Bennett and New Jersey Hospital Association (NJHA) President and CEO Betsy Ryan will unveil New Jersey’s new electronic POLST form March 31 at Villa Marie Claire Hospice in Saddle River.

The POLST – Practitioner Orders for Life-Sustaining Treatment – form, which empowers individuals to make decisions regarding their medical care at the end of life, was released in 2013. The electronic POLST makes this form more accessible online via a secure website and mobile devices.

These forms are designed to be completed jointly by an individual and a physician or advance practice nurse, detailing the individual’s goals of care and medical preferences. The POLST form is intended for patients with a life-limiting illness; it allows them to state detailed preferences on specific goals and medical interventions.

Joseph Carr, NJHA Chief Information Officer, will demonstrate how the electronic POLST can be accessed and how it can help inform care.
