Department of Health Announces $250,000 Grant Opportunity to Integrate Health Care for Military, Veterans, and First Responders

The New Jersey Department of Health today announced a $250,000 grant opportunity to support the creation of a new pilot program or for support of an existing program to integrate primary care, behavioral health, palliative care, hospice, and telehealth services for members of the military, veterans, and first responders. This one-year grant opportunity is available to New Jersey hospitals, healthcare systems, federally qualified health centers, and state university providers of behavioral health services.

Military personnel, veterans, and first responders often deal with primary care and behavioral health challenges. Offering the integration of healthcare across the continuum will help create a seamless system of care.

“Our military, veterans, and first responders have consistently put their lives at risk to protect all of us. They deserve integrated healthcare services, not only to pay back this debt, but also as part of the central mission of defense and preparedness,” Health Commissioner Shereef Elnahal said. “The Department believes that this investment will ensure that care and services are accessible and coordinated in a comprehensive and efficient way.”

The goal of this grant is to support further development of integrated care networks in New Jersey that can serve the ongoing primary and behavioral health care of military, veterans, and first responders.

A letter of intent to apply is due by 2 p.m. December 7, 2018.  Applications are due by 3 p.m. December 24, 2018.
