The Open Public Records Act (OPRA) requires that every public agency adopt the GRC's official OPRA records request form. N.J.S.A. 47:1A-5(f).

Custodians seeking to adopt the GRC's established OPRA request form must access the "Adtopable OPRA Request Portal" at Government Records Council (GRC) | Adoptable OPRA Request Form (nj.gov). 

 

Given this new requirement, requestors may wish to review the website for the agency to whom they intend to submit an OPRA request. That agency's website should have a copy of their established OPRA request form (with relevant contact information) and information on how to submit an OPRA request.
