Program Description

Each of New Jersey’s 21 counties maintains a Special Child Health Services (SCHS) Case Management Unit (CMU). These programs are funded by the New Jersey Department of Health and each county’s Board of Commissioners. Their mission is to promote access to preventative and primary care for children and youth with special health care needs from birth up until their 22nd birthday.

Goals

The goal of SCHSCM is to provide comprehensive, coordinated, culturally competent case management services for families with CYSHCN.
