Community-Based Palliative Care is a benefit available to NJ Family Care members, starting April 1, 2026. The benefit was signed into the law by Governor Murphy on December 21, 2023, underscoring the state's commitment to supporting residents facing serious illness.

Community-Based Palliative Care (CBPC) is designed to support people living with serious health conditions by helping manage symptoms, coordinating care, and making the healthcare system easier to navigate. You can receive CBPC while continuing your current treatments. CBPC is not hospice; a terminal diagnosis is not required. It is designed to give you extra support alongside the care you already receive.  

CBPC gives you access to a team of professionals who work with you and your doctor to understand your needs, your goals, and what matters most to you. Together, they create a personalized care plan and provide services in the community, including at home and in other non-hospital settings.

Information for NJ FamilyCare Members interested in receiving Community-Based Palliative Care

Managed care members

If you are a managed care NJ FamilyCare member looking for Community-Based Palliative Care, call your managed care “Member Services” number (listed here) and ask for the Community-Based Palliative Care team for help. To find the CBPC directory for your specific managed care plan, please see:

Fee-for-service members

This benefit is available to both Managed Care Organization (MCO) and Fee-For-Service (FFS) members. Members may be referred for a CBPC assessment by any healthcare provider. To broaden access, DMAHS offers an optional, brief Program Eligibility Screening Tool that may accompany referrals. After referral, a clinician or provider entity must complete a reimbursable eligibility assessment using the DMAHS-developed standardized Comprehensive Medical Assessment Tool and submit to MAHS.CBPC@dhs.nj.gov

 
 
Information for providers interested in enrolling in NJ Family Care

We welcome providers that have the necessary requirements to enroll as NJ FamilyCare providers for Community-Based Palliative Care services. 

To see if you meet NJ FamilyCare’s CBPC requirements, see our CBPC policy guidance for more information.

Starting February 2nd, 2026, providers who meet the requirements are invited to:

  • Enroll in the NJ Medicaid Management Information System (NJMMIS)
  • Once enrolled in NJMMIS, you may complete credentialing and contracting with any or all of NJ FamilyCare Managed Care Organizations (MCOs) to deliver and bill for CBPC services. Providers are encouraged to participate with all MCOs.
