The National Association of Insurance Commissioners (NAIC) website is the source for uniform state requirements and forms for Primary, Expansion, or Corporate Amendments applications.

To file an Expansion or Corporate Amendments application, visit the Uniform Certificate of Authority Application (UCAA) system website.
 
UCAA and State Specific Forms

Expansion Applications:
Life/Health Insurance Companies


Non-Life Insurance Companies

Foreign Insurers seeking to obtain a Certificate of Authority must submit an Application for expansion through the UCAA website and complete the General Eligibility Worksheet.


Adding or Deleting Lines of Business:

Life/Health Insurance Companies


Non-Life Insurance Companies


A New Jersey registered insurer seeking an extension of authority must submit a Corporate Amendments Application through the UCAA. You may visit the NAIC Home website before filing, for information concerning forms and uniform state requirements.


Primary Applications:

Life/Health Insurance Companies

Non-Life Insurance Companies

Newly formed insurance companies must first submit a feasibility study. Refer to the Letter w/Instructions below.

The NAIC Home, Primary App tab, provides a listing of the relevant forms. Newly formed domestic insurers may not be able to file electronically through the UCAA website.

Re-domestication to New Jersey - a company must follow the above-mentioned state specific requirements. The insurer may be able to file their Primary Application through the UCAA website, since it is an established insurer.

OPRA is a state law that was enacted to give the public greater access to government records maintained by public agencies in New Jersey.
