Registration Forms for Attendance Based on Student Residency

Districts are not required to use the provided registration forms. For additional information, refer to the Requirements for Locally Developed Registration Forms section.

Instructions

  1. Read the information on the Attendance at School Based on Domicile or Residency in the School District page.
  2. Complete the appropriate form according to the situation that best matches the student’s circumstances, as described below:

Section D—Special Circumstances

Complete the Section D: Special Circumstances form if any of the following apply:

  1. The student is the child of a parent or guardian who has moved to another school district as the result of being homeless.
  2. A court order has placed the student in the home of a school district resident who is not the student’s parent or guardian.
  3. The student has been placed in the school district by the Department of Children and Families acting as the student’s guardian.
  4. The student’s parent or guardian:
    1. previously resided in the school district;
    2. is a member of the New Jersey National Guard or the reserve component of the United States armed forces; and
    3. has been ordered to active service in the United States armed forces in time of war or national emergency, resulting in the student’s relocation.
  5. The student is kept in the home of a person domiciled in the school district who is not the student’s parent or guardian, and the parent/guardian:
    1. is a member of the New Jersey National Guard or the reserve component of the United States armed forces; and
    2. has been ordered into active military service in the United States armed forces in time of war or national emergency.
  6. The student resides on federal property.

Requirements for Locally Developed Registration Form

Any locally developed registration form:

  • must include the preliminary information found in the preliminary information section; and
  • may not request information of the type prohibited by N.J.A.C. 6a:22-3.4(d) as indicated in the Preliminary Information section.
