What is USTF+ Reporting
The Unified Service Transaction Form (USTF+) is the data collection mechanism for the Division’s Client Registry, which is our system-wide database for information concerning the characteristics of clients served in NJ’s publicly-funded mental health system. Information from this system assists the Division’s planning and program developments. It is also used to determine incidence and prevalence rates, the number of persons with serious and persistent mental illness and unduplicated counts of persons served. More Information
