The Critical Incident Management Unit (CIMU) manages the Unusual Incident Reporting & Management System (UIRMS).

Unusual Incident Reporting & Management System (UIRMS)...

UIRMS is an electronic way of collecting, reporting, and analyzing information about unusual incidents that occur in Department of Human Services (DHS) and the Department of Children and Families (DCF) facilities and contracted programs.


Unusual incidents involve a service recipient and are occurrences which may potentially have an adverse impact on the health, safety and welfare of that service recipient or others.


The UIRMS system lets staff inform management of incidents more quickly; respond more quickly to incidents, speed up the investigation response, and then resolve and analyze the data for statistical and quality assurance (such as tracking/trend analysis and proactive action) purposes.


This system can generate reports which will help the Departments and Divisions enhance their efforts to improve the quality of services provided to all of our many consumers/service recipients.

CIMU Mission...

CIMU’s mission is to facilitate the appropriate tracking, management and organizational response to all reported Unusual Incidents occurring in facilities and programs under the purview of DHS and DCF.  

In addition, CIMU seeks to assure protection from harm for all DHS Service Recipients through the related analysis and reviews of all reported Unusual Incidents, in support of DHS objectives.

Unit Functions...

The Critical Incident Management Unit

  • Administers the Unusual Incident Reporting and Management System (UIRMS);
  • Processes, monitors, tracks, and analyzes all aspects of Unusual Incident Reports (UIRs) via the UIRMS system ;  
  • Assigns UIRs to other offices for investigation, evaluation, and closure responsibility ;
  • Conducts the Administrative Review process which reviews some contracted provider investigation reports from the Division of Developmental Disabilities (DDD) ;
  • Analyzes, trends, and presents data during the Comparative Statistics (COMPSTAT) review process; and
  • Manages and maintains the OPIA Risk Management System Risk Indicator Report.
Administrative Review Process...

The Critical Incident Management Unit is responsible for reviewing and processing contracted provider investigation reports that involve some allegations of abuse, neglect, and exploitation. The contracted providers covered by this process are from the DDD community.

CIMU reviews completed Agency Investigation Reports for quality assurance purposes, then closes the incident on behalf of the Department.

Investigations are to be submitted within 30 days of the incident being reported.  Then the CIMU staff completes its review within 30 days of receiving the report, only advising the agency of any deficiencies found.

CIMU records reflect that the investigation report required to close the incident has not been submitted within the required 45 days, a letter will be sent to the contracted provider indicating that they have ‘Failed to Comply’ with the requirements of Administrative Order 2:05 and the community addendum. 

Comparative Statistics (COMPSTAT) Process...

The COMPSTAT (Comparative Statistics) model is a proven vehicle for measuring the effectiveness of a system's operations.

CIMU conducts regular reviews of the critical issues surrounding the Department's management of Unusual Incidents… reviewing the Unusual Incident Information, Data Trends, Patterns, Actions initiated and finally, the Outcomes.

The purpose of COMPSTAT is to gather, organize and present critical incident reporting information for DHS facilities and community residences.  The information is tailored to the concerns, questions, and responsibilities of the DHS Division.

The COMPSTAT review establishes and promotes communication, collaboration, action, accountability, and the sharing of best practice strategies between the OPIA Offices and DHS Divisions.

This review process also results in recommendations to the division, agency and/or service provider for Plans of Improvement /Action Plans and/or Specific Interventions, based upon data identified and initiated by COMPSTAT.
