The New Jersey Department of Military and Veterans Affairs has partnered with Unite Us to connect individuals and families to a wide variety of services and resources. With a commitment to utilize and leverage our own and external resources for the betterment of New Jersey's military/veteran community, our partnership with Unite Us allows us to improve the health and well-being of the people we are committed to serving.

Unite Us is a coordinated care referral network of health and social service providers. Unite Us can connect our New Jersey military/veteran community to resources to obtain benefits, food assistance, housing, education, transportation, behavioral health services, peer support and more.

If you are experiencing a non-urgent need and would like to be connected to services appropriate for you, please select the link below. Once the Assistance Request Form has been submitted, a Unite Us Care Coordinator will contact you - normally within two business days.

The information you enter is completely confidential and will only be shared to connect you to services. There is no cost for this service.


Consent to Participate in the Unite Us Network

By consenting, you agree to share information with a Network of health and social service partners powered by Unite Us software. This network is made up of entities and individuals who are directly involved in your care or payment of care. Your personal information may be shared securely on the Network in accordance with privacy laws to connect you with services.

This consent covers all information shared by you or by anyone that has the right to share information on your behalf and is relevant to the recipient's involvement in your care or payment for your care. You can always limit the information you provide on the Network by requesting to have it removed.

Data Protection

The State of New Jersey has a number of security requirements and technical assessments that must be accomplished as part of the platform’s use and activation. Coordinated through New Jersey’s Office of Information Technology, requirements and assessments span physical, administrative, and technical controls such as encryption and vulnerability monitoring. Multiple federal and state regulations apply including HIPAA, 42 CFR (Code of Federal Regulations) Part 2, and FERPA (The Family Educational Rights and Privacy Act).
