File a Complaint

If you would like to file a complaint regarding abuse, neglect and/or quality of life at a long-term care facility, you can contact the NJ Long Term Care Ombudsman and/or the NJ Department of Health.

If you suspect Medicaid fraud, waste or abuse you may file a complaint with the Office of the State Comptroller's Medicaid Fraud Division and/or NJ Medicaid Fraud Control Unit.
