Community Care Program Waiting List

Individuals determined eligible for Division services can enroll and begin receiving services in the Supports Program at any time.

The Community Care Program (CCP) waiting list is a tool that enables individuals and their families to notify the Division of a potential need for CCP enrollment. 

However, addition to the list is not a guarantee of CCP enrollment. Determination of eligibility and need for CCP enrollment can be made at the time an individual is reached on the waiting list. For CCP enrollment, in addition to meeting all other Division eligibility requirements, an individual must be determined by the Division to require an ICF/IID Level of Care (Intermediate Care Facilities for Individuals with Intellectual Disabilities) This means that, but for CCP enrollment, the individual would qualify to receive services in a State run institution for persons with intellectual and developmental disabilities. 

The standards for addition to the CCP waiting list and the procedures for CCP enrollment are found in Division Circular 8 (N.J.A.C. 10:46C)(pdf).  In general, the Division works through the list chronologically.

Individuals may request addition to the CCP waiting list if they are:

OR if they:

  • Were born prior to January 1, 1997,
  • Were determined by the Division to be eligible for services prior to January 22, 2013, and
  • are Medicaid eligible.
