Check Eligibility

The I Choose Home program serves only individuals who are Medicaid eligible and have resided in a nursing facility 60 days or more. If the individual seeking services is in the process of applying for Medicaid, please reach out to us again after you or your loved one is found eligible for Medicaid.

While I Choose Home criteria was not met at this time, we recommend you or your loved one:

  • Speak with the facility social worker about their interest in living in the community.
  • Request that their interest in living in the community is documented in the plan of care.
