The data in the following tables are from the Annual Immunization Status Reports (ASR). The ASR is completed by each individual school in New Jersey. Please keep in mind the following limitations when reviewing these data:

  • The data are self-reported by the schools and are not verified by DOH or the local health department.
  • Although submission of the ASR by schools is required, not all schools comply; submission rates vary year-to-year.
