State Health Benefits Program

Eligibility for Active Group coverage is determined by the State Health Benefits Program (SHBP). All applications to enroll, change coverage, terminate, etc. must go through your employer.

Full-time Employees

  • State Employees — To be eligible, you must be a full-time employee of the State of New Jersey or be a full-time appointed or elected officer of the State (this includes employees of a State agency or authority and employees of a State college or university). To be considered full-time you must work at least 35 hours per week.
  • Local Government Employees — To be eligible, you must be a full-time employee or a full-time appointed or elected officer from a local employer that participates in the SHBP (county, municipality, county or municipal authority, etc.). Each employer determines the minimum hours to be considered full-time by a resolution filed with the NJDPB; however, it cannot be less than 25 hours per week for an employee or less than 35 hours per week for an appointed or elected official who becomes eligible after May 21, 2010. Employment must also be for 12 months per year.

State and Local Government Employee Applications — All State and Local Government Employees MUST submit Health Benefit applications online using Benefitsolver. Log in through myNewJersey or navigate to mynjbenefitshub.

All covered employees must have access to prescription drug coverage. The type of coverage provided and costs are determined by the option offered by your employer and the medical plan you select.

The New Jersey State Health Benefits Program (SHBP) and School Employees' Health Benefits Program (SEHBP) offer employees and their covered dependents the opportunity to join a Direct Primary Care doctor's office at no additional cost. Simply choose a Direct Primary Care Doctor from Marathon Health (formerly Everside Health) and get unlimited access to personalized care with no co-pays, when and where it's convenient for you. NJWELL is a wellness program offered to active employees of the public sector who participate in the State Health Benefits Program (SHBP) or School Employees’ Health Benefits Program (SEHBP) and their covered spouses/partners. Learn more about NJWELL.

Eligible active and retired members and their dependents can take advantage of these add-on health programs (copays or additional out-of-pocket costs may apply).

  • Forge  – Active, COBRA, and non-Medicare retirees enrolled in a Horizon medical plan who are 18 years of age or older can start their recovery journey with a personalized behavior health plan to help you achieve your goal.
  • AbleTo for Horizon members and AbleTo for Aetna members – Certain active, COBRA, and non-Medicare retirees who are enrolled in a Horizon medical plan or the Aetna Garden State Health Benefits Plan can get a personalized, behavior health-focused 8-week program to learn skills to help you enjoy more out of life.
