Community-based support coordination agencies and the support coordinators they employ help manage care for people who receive DDD-funded services. They work independently from service providers.
Support coordinators work with you and your planning team to create your yearly service plan. They also update the plan whenever changes are needed.
How Your Support Coordinator Stays In Touch
Your support coordinator will meet with you at least once a month — either in person, by phone, or by video call. During these check-ins, they will:
- Make sure you are healthy and safe
- Confirm that your services are still working well for you
How to Choose a Support Coordination Agency
You can use DDD's online Provider Search Tool to find support coordination agencies that serve the county where you live.

- View or download a list of agencies as a PDF. This list includes up-to-date details pulled from iRecord, DDD's online care management system.

How to Change Your Support Coordination Agency
If you are enrolled in a DDD Medicaid waiver program (Supports Program or Community Care Program), you have the right to switch to a different support coordination agency.
To make a change:
- Fill out the Support Coordination Agency Change Form.docx.
- Send it to DDD.
Support coordination agency changes take effect at the beginning of the next month.
