Application for Determination of Eligibility

To find out if you are eligible for DDD services, you need to complete an Application for Determination of Eligibility. There are two versions — a full application and a short application.

A family member or someone else can help you fill out the application, but they cannot submit it on your behalf. The application must come from you.

After we receive your completed application and all required documents, it can take up to 60 days for DDD to let you know if you are eligible.

If your application is missing information or required documents, it may take longer.

Should I use the full or short application?

To find out if you need to use the full application or you can use the short application, ask yourself these questions:

  1. Have you applied for developmental disability services in New Jersey?
    1. If YES, go to question #2 below.
    2. If NO, you need to use the full application.
  2. Were you determined eligible for developmental disability services through the NJ Children's System of Care (also known as PerformCare)?
    1. If YES, you can use the short application.
    2. If NO, go to question #3 below.
  3. Were you determined eligible for developmental disability services through the Division of Developmental Disabilities (DDD)?
    1. If YES, you can use the short application.
    2. If NO, you need to use the full application.

Submitting the completed paper or PDF application

You can submit your completed Application for Determination of Eligibility (full or short) by email, mail, or in person. Be sure to include all of your supporting documents at the same time you submit your application.

Submitting Application by Email

  • Scan the completed and signed application, as well as all supporting documents, into your computer. Submit them by email, as an attachment, to DDD.NJApply@dhs.nj.gov.
  • Enter in the Subject Line: Intake Application - Individual's Initials - County of Residence (Example: Intake Application - JS - Mercer County).

Submitting Application by Mail

  • Mail your application and supporting documents to DDD Intake.
  • Use the address of the Community Services Office for the county where the person with disabilities lives.

Submitting Application In Person

  • Hand-deliver your application and supporting documents to the Community Services Office for the county where the person with disabilities lives.
  • Community Services Offices are open from 9 am to 5 pm, Monday through Friday.
