Waiver Programs
As part of the New Jersey FamilyCare Comprehensive Demonstration, authorized under Section 1115 of the federal Social Security Act, DDD administers two Medicaid home and community-based services (HCBS) waiver programs: the Community Care Program and Supports Program.
To get services, you need to be enrolled in one of these programs. While they share basic eligibility rules, the Community Care Program has additional eligibility criteria.
There is no waiting list to enroll in the Supports Program. To enroll in the Supports Program and begin receiving services, a person must:
- Be determined through the DDD intake and application process to meet the functional criteria for a developmental disability as defined in Division Circular 3 (PDF)
- Be aged 21 or older
- Be a legal United States citizen and a resident of New Jersey
- Be eligible for or enrolled in Medicaid
Right now, there is a waiting list to be evaluated for the Community Care Program.
To enroll in the Community Care Program and begin receiving services, a person must:
- Meet the basic program eligibility rules above
- Be reached on the waiting list or be determined by DDD to be at imminent risk of homelessness
- Be determined by DDD to meet the criteria for "institutional" Level of Care (LOC)
- Meet specific Medicaid income and resource criteria

Program participants work with their support coordinator and planning team to choose services based on their assessed needs and life goals.
